Scotland Health is proud to share that Dr. Shelly Lowery, Chief Medical Officer and Senior Vice President of Ambulatory Practices, served as a featured speaker at the 2026 North Carolina Community Health Center Association (NCCHCA) Rural Primary Care Conference, held June 3–5 at the Washington Duke Inn in Durham, North Carolina.  

Dr. Lowery’s presentation, “Scotland Health at Home: Our Approach to Care Management and Community Health Workers,” highlighted Scotland Health’s innovative model for extending care beyond traditional clinical settings to better serve patients across rural communities.  

The conference followed the State Designated Rural Health Centers (SDRHC) End of Year Meeting held on June 3, where healthcare leaders from across North Carolina gathered to collaborate, share best practices, and strengthen rural healthcare delivery.  

During her presentation, Dr. Lowery emphasized how the Scotland Health at Home program supports physicians and advanced practice providers (APPs) in delivering comprehensive, whole-person care. The program is designed to ensure patients receive ongoing support not only during clinic visits or hospital stays, but also within their homes and communities.  

By meeting patients where they are, the program addresses real-world barriers that impact health outcomes, while also strengthening provider engagement and improving the overall health of the populations Scotland Health serves.  

“Programs like Scotland Health at Home reflect our commitment to patient-centered care,” said Dr. Lowery. “By extending care into the community and integrating resources such as community health workers, we are able to better support our patients in managing their health and improving their quality of life.” 

Scotland Health’s approach integrates care teams that include community health workers and other support services to help patients navigate challenges such as transportation, access to food, and follow-up care—further reinforcing the organization’s commitment to improving health outcomes across southeastern North Carolina.  

Leadership from Scotland Health’s Population Health team, including B.J. Goodridge, MHA/INF, CSSGB, CRHCP, Population Health Manager, and Jamie Cicali, MSW, NHA, Executive Director of Population Health and Continuum of Care, also attended and supported the presentation, demonstrating the organization’s collaborative approach to advancing rural healthcare innovation.
