Infusion Services

Scotland Outpatient infusion services allow medications to be delivered intravenously (through an IV) or by an injection.

How to make a referral

To make a referral, providers can select the drug of choice from the list below, complete the form and fax the completed form to (910) 291-7065.

Please include with referral:

Demographics sheet
Supporting documentation (clinical progress notes)
- Any tried and/or failed medications (if applicable)
Relevant lab work
Signed MOU (memorandum of understanding)

A nurse will then contact the patient by phone to schedule an appointment.

Location

The Infusion Clinic is located at 500 Lauchwood Drive in Laurinburg inside of the urgent care building.
