At Scotland Health, we recognize that transportation has consistently been one of the biggest barriers to accessing healthcare in the communities we serve. Our recent Community Health Needs Assessments have highlighted this ongoing issue, and we are committed to taking action to address it.
Our Population Health Team has implemented several initiatives aimed at improving access to healthcare and bringing services closer to those who need them most:
Thanks to the generous donations from our foundation-sponsored Ritz Gala, we are proud to offer transportation vouchers for patients who require assistance getting to their medical appointments. In November, our Foundation raised nearly $70,000 through our Fund the Need campaign, which supports the basic needs of patients, including transportation.
In partnership with Pembroke Rescue Squad, we have launched an innovative in-home care program that delivers healthcare directly to patients. This initiative not only reduces the need for travel but also significantly improves health outcomes for those we serve.
We have expanded our Community Health Worker (CHW) team to five dedicated members. These trusted professionals assist patients in navigating the healthcare system, helping them find the right resources and education tailored to their specific needs and income levels.
We are actively addressing health equity by working to reduce transportation barriers for lower-income and minority residents in our county. Our efforts ensure that all members of our community—regardless of their background or economic status—can access the care they need.
